This piece is definitely transformative with the horror aspect, but I don't quite understand how the bloody handprint is working in the scene floating in thin air).
This piece is spooky! The transformation of this portrait really carries that theme through. The only thing that I might change would be to create some sort of obvious surface where the bloody hand print is on. So if you could maybe add a glass wall or something to convey that, the idea of this piece would be stronger.
All the changes to the work make the scene more dramatic and disturbing. Your definitely headed in the right direction. The change in color and the basic composition with the woman in the lower part of the frame also complement the idea. I would consider modifying the background a little further by increasing the contrast and pulling out all the color in the pine trees. Basically, just make it a little more stark.
I am very impressed with the diegetic verisimilitude of the wounds on the figure. The way they adhere to the contours of the form beneath them, behaves in such a way that regardless of actual realism feels real in such a visceral and gut wrenching way. And the rest of the edits are so competently executed that the whole effect is sold.
If I had one complaint, and this is reaching, it would be that the final filter removes some of the contrast of the original photograph.
